Sports & School Physicals
Sports, School & Work Physicals in the Bronx, NY
A sports or school physical is a short exam that confirms it's safe for you to play, attend or work, and it ends with a signed form. It's not the same thing as a full annual physical, though the two can often be combined in one visit.
Who this is for
- Students who need a school, camp or sports clearance form signed
- Adolescent athletes before a season starts
- Adults who need a pre-employment physical for a new job
- Anyone whose employer requires an annual fitness-for-duty exam
- Parents who want the school form and the yearly checkup handled together
What happens at the visit
Bring the form
Bring the exact paperwork your school, league or employer gave you. If part of it has to be filled out by a parent, do that section before you arrive.
History
Past injuries, concussions, asthma, allergies, medications, and any family history of heart problems or sudden death — this section matters most for athletes.
The exam
Vision, height, weight, blood pressure, heart and lungs, abdomen, joints, flexibility and strength.
Extra testing when indicated
If the history or exam raises a question about the heart, your doctor may recommend an EKG, which can be done at the office.
Signed and done
In most cases the form is completed and signed at the visit and you take it with you. Ask for a copy for your own records.
Ready when you are
Calling is the fastest way to be seen. Our staff answers in English or Spanish.
What to bring

Insurance and cost
Many plans cover a school or sports physical as part of a preventive visit. Employer-required exams are sometimes the responsibility of the employer or the patient.
We accept Medicaid and most managed Medicaid plans, along with many commercial plans. Coverage rules change often, so call (917) 801-0954 with your card in hand and our staff will confirm what your plan covers before your visit.
Self-pay patients are welcome. Ask our front desk what a visit will involve when you call so there are no surprises.

Frequently asked questions
- Can I get the form signed the same day?
- In most cases yes, as long as you bring the form with you and the exam doesn't turn up something that needs further testing.
- Is a sports physical the same as an annual physical?
- No. A sports physical focuses on clearance to participate. An annual physical is a broader preventive exam, and the two can often be done together.
- What age patients do you see?
- Call (917) 801-0954 and our staff will confirm the ages we see for this visit.
- Do you do pre-employment physicals?
- Yes. Bring the employer's form and any specific tests they've requested.
- Does my child need to bring a parent?
- For patients under 18, a parent or guardian should come to sign consent and any parent sections of the form.
