Privacy Policy and Notice of Privacy Practices
What we collect on this website, how we use it, and the rights you have over your medical information.
This website
This site is informational. It does not ask for symptoms, diagnoses or medical history. The appointment request form collects only your name, phone number, email, preferred language, the type of visit, your preferred day and time, and whether you are a new or existing patient — enough for our staff to call you back and book the appointment.
Please do not send medical details through this website or by email. A public web form and ordinary email are not secure channels for protected health information. We will collect anything clinical securely when we speak with you.
How we use what you send
- To contact you about the appointment you requested.
- To confirm your insurance coverage before your visit.
- To answer a question you asked us directly.
We do not sell your information. We do not share it with advertisers.
Your rights as a patient
Health information created when you are treated at NorthPoint Medical Associates is protected by the federal HIPAA Privacy Rule and by New York State law, which is especially strict about HIV-related information. As a patient you generally have the right to:
- See and get a copy of your medical record.
- Ask us to correct information you believe is wrong.
- Ask us to communicate with you in a specific way or at a specific address.
- Ask for a list of certain disclosures we have made.
- Ask us to limit what we share, and get a paper copy of this notice.
- File a complaint if you believe your privacy rights were violated.
To exercise any of these rights, call (917) 801-0954 or write to us at 1301 Southern Boulevard, 2nd Floor, Bronx, NY 10459.
