Children's Physical Exams

Children's Physical Exams in the Bronx, NY

A children's physical is the exam a school, camp, sports team or program requires before a child can take part. It is also a chance to check growth, vision, hearing and vaccinations while the whole family is already in the office.

Call (917) 801-0954

Who this is for

  • Children and adolescents who need a school or daycare physical form completed
  • Young athletes who need a sports clearance before the season
  • Camp and after-school program enrollment
  • Families who want siblings seen in the same visit as a parent's appointment
  • Children who need vaccinations brought up to date for school

What happens at the visit

  1. Bring the form with you

Every school and program uses its own form. Bring the exact one your child's program requires and we complete it during the visit.

  2. Growth and vitals

Height, weight, blood pressure and pulse are recorded, along with vision and hearing screening where the form asks for it.

  3. The examination

A head-to-toe exam covering heart, lungs, abdomen, spine, joints and skin, with attention to anything that would affect safe participation in sports.

  4. Vaccinations if they are due

School-required vaccinations can usually be given at the same visit so you do not have to come back.

  5. The form goes home with you

The completed, signed form is handed to you at the end of the visit unless the program requires it to be sent directly.

Frequently asked questions

What ages do you see?
Call the office and our staff will confirm the ages we see for physical exams.
Can the form be completed the same day?
Yes. Bring the form to the appointment and it is completed and signed before you leave.
Does a parent have to be present?
A parent or legal guardian should come with a minor child, both for consent and because the medical history matters.
My child needs shots for school. Can that happen in the same visit?
In most cases, yes. Bring the immunization record so we can see exactly which vaccines are due.
Do you speak Spanish?
Yes. The entire visit, including the form and the instructions, can be handled in Spanish.
